Report of Marriage

My passport is expiring next year and I really have to get it renewed and amended. But before I send the passport application, I needed to have the report of marriage because it is one of the Passport Application requirements. So, we filed our report of marriage last month and we got it back 7-8 days after we sent it to the Philippines Embassy in Washington DC since the place of our marriage took place on its jurisdiction which was South Carolina. It was pretty easy and fast. We just sent the following requirements.

•Four (4) original Notarized copies of the Report of Marriage Contracted Abroad form which was signed by me and my husband.

•Four (4) identical colored 1.77 in. x 1.37 in. photos each of me and my husband with plain white background.

•One (1) original copy and four (4) photocopies of our Marriage Certificate.
(They sent us back the original copy of our Marriage Certificate.)

•One (1) Information Sheet on Filipinos Marrying Foreign Nationals form signed by me.

•A money order worth $25 payable to the Embassy of the Philippines.

•We also enclosed a pre-stamped, self-addressed U.S. Postal Service Express Mail envelope with stamps required to send 0.5 lbs. of documents.


So, 7-8 days after we sent it in, we got our Report of Marriage form back. The one that we filled up , notarized and signed and this time, it’s got a signature of a Consular Officer and a seal on it.
